Congenital intestinal malrotation is a gastrointestinal anomaly whose most serious complication is midgut volvulus. More commonly, it presents as an incidental finding at laparotomy, or as a finding on diagnostic imaging (Ultrasound, CT, Upper GI contrast study). Most patients are diagnosed in childhood. Laparoscopic Ladd's procedure is an accepted alternative to Laparotomy in children but has not been well-studied in adult. We
present the case of this unexpected finding in a patient 38 years old, during emergency laparotomy for mechanical intestinal obstruction. Intraoperative findings included intestinal malrotation with small bowel volvulus. The terminal ilea and cecum were gangrenous on the basis of ischemic necrosis. A limited right hemycolectomy and primary end-to- end anastomosis was performed.

Perianal malignant nodular hidradenoma in HIV infected pregnant patient

Malignant nodular hidradenoma (MNH) is a rare invasive tumour arising from eccrine sweat glands, with significant risk for local recurrence and distant metastases. Hidradenoma papilliferum is reported in pregnancy. To our knowledge this is the first MNH in a human immunodeficiency virus (HIV) infected pregnant woman. The patient presented with a painless perianal growth. Wide local excision was performed and the wound left to heal by secondary intension. The pathology report confirmed MNH. The patient has been followed for 14 months without evidence of recurrence. MNH should be considered in the differential diagnosis of similar lesions in HIV infected patients. Treatment for such HIV infected patients with MNH may be similar to that in immune-competent patients; they may benefit from a longer follow-up period.

Summary: We report the case of a 39-year-old HIV positive female with gastric outlet obstruction. At laparotomy, she had extensive pneumatosis intestinalis (PI) of the ileum. This incidental finding of extensive PI in an HIV positive patient who was being managed non-operatively supports that the mere presence of PI is not an absolute indication for bowel resection in the absence of necrosis, perforation, or obstruction.

Segmental atrophy of the liver in a child: case report and review of the literature

Segmental atrophy of the liver (SAL) is a pseudotumour that is extremely rare in children. We report the case of a 4-year-old child who presented with abdominal pain and a non-tender mass at the epigastrium. A computed tomography (CT) scan revealed a complex lesion of the left lobe of the liver. At laparotomy, a mass arising from the caudate lobe was completely excised and the pathology report confirmed SAL. Although rare in children, paediatricians, surgeons and radiologists should consider the possibility of SAL in the differential diagnosis of hepatic masses in this group of patients.
